Forms & Waivers
Fitness & Performance
Training Waiver
For adults and athletes participating in fitness or performance training at Invictus. Please complete your waiver before your first session. For participants under 18, a parent or legal guardian must complete the waiver.
Patient Forms
After you schedule your appointment, we'll send your patient forms securely. Please complete them before your visit so your provider can review your health history, concerns, and goals. If you haven't received your forms or need help completing them, contact our office.
Before Your First Visit
Coming in for care?
Things to bring to your first visit:
A photo ID.
Your insurance card, if you plan to use insurance for an eligible service.
A current list of medications and supplements.
Relevant imaging reports, lab results, or surgical instructions, if available
Your first clinical visit typically includes a discussion of your concerns and goals, an evaluation, and recommendations for next steps. Treatment may begin that day when appropriate.
Coming in for training?
Wear clothes you can move in and athletic shoes, bring water, and complete your training waiver before your first session.
Let your coach know about any injuries, activity restrictions, or concerns that may affect your workout.
Payment & Insurance
Payment options vary by service. We offer insurance billing for eligible clinical services and self-pay options. Hormone and health optimization services are cash-pay.
Our office can help you understand payment options and request a benefits check before your visit. Insurance benefits vary by plan, and verification does not guarantee payment.
